Player Waiver

Participation in the activities of Western Nevada County Recreation Services (WNCRS) is inherently hazardous. Softball is strenuous and may be affected by adverse weather conditions. Discomfort, injury, and in some cases, death may result. It is recommended that all participants should carry their own insurance coverage as WNCRS DOES NOT provide it. Each participant is assuming all risks, consequences, and potential liability.

I, the undersigned, certify that I am in good health and able to participate in this event. I assume and agree to hold harmless WNCRS, its members, organizers, sponsors, and any other person, firm, or corporation from any and all claims directly and indirectly resulting from participation in softball play offered under the auspices of WNCRS. I fully understand the scope of this activity and I voluntarily sign this form.
