Liability Waiver
Participation in the activities of the Western Nevada County Slo-Pitch Softball Association (WNCSSA)/Western Nevada County Recreation Services (WNCRS) is inherently hazardous. Softball is strenuous and may be affected by adverse weather conditions. Discomfort, injury and in some cases, death may result. It is recommended that all participants should carry their own insurance coverage as WNCSSA/WNCRS does not provide it. Each participant is assumed to be voluntarily performing activities for which he/she assumes all risks, consequences and potential liability. I, the undersigned, certify that I am in good health and able to participate in this event. I assume and agree to hold harmless WNCSSA/WNCRS, its members, organizers, sponsors and any other person, firm or corporation from any and all claims directly and indirectly resulting from participation in softball play offered under the auspices of WNCSSA/WNCRS. I fully understand the scope of this activity and I voluntarily sign this form.
